Basic Activity Waiver
Description
This is a template for a basic activity waiver. This document is designed for businesses and/or non-profit organizations that will be providing in-person services, activities, or events. The goal of this agreement is to ensure that participants understand the risk inherent in participating in these activities and, therefore, agree not to hold your organization liable if they are injured as a result of such voluntary participation. You will want to customize the waiver to mention the specific activities you are offering (yoga, sports training, sports league, volunteer activities, etc.) and if there are any specific risks as well.
